FALL PROTECTION PLAN

Fall Protection Plan is required for work above 8 ft.  This document must be provided by the contractor or downloaded in the doc-shop. Work may not proceed until this document is completed.  All workers must review and sign.  ALL workers MUST have a certification for fall protection a copy is required on-site at all times during the work and perform an inspection of equipment prior to use [use fall protection check list].  All workers  must sign the competency form for Fall Protection if a copy of the certification is not on site  signed off by the owner of the company.  Download the Fall Protection Check List.  This is a critical high-risk task.  A certified level 3 OFA first aid is required on-site during all high-risk work hours.
