HEARING TEST COMPETENCY FORM

ALL workers are required by governing authorities to have a yearly hearing test and must produce a hearing test card.  When your contractor tells you they have records in their office confirm in writing with this liability form transfer to your contractor.  Keep your file onsite or readily available.  A copy must be present on site at all times. 1 incident can risk your investment or to be covered by your brokers.
